What is available under the gem three-dot menu?

The gem menu includes history, Gem Settings, the project optimizer, recently closed shard recovery, and a per-gem approval shortcut. Some rows appear only when they apply. The Gem Settings panel has general, agents, and worktrees tabs. See the gem menu.
